For our first visit to your Thornhill dental clinic, what do we need to bring with us?

For their first visit we recommend you arrive about 15 minutes prior to your appointment as we will need to you fill out our patient information forms. Please be sure to bring your child's health card and proof of insurance if you have any. Lastly, make sure to bring a SMILE!
